Overview

JMU IT provides three survey tools:

Microsoft Forms: Web-based survey tool for general use and available in Microsoft 365.

  • Microsoft Forms is not approved for the collection of highly confidential data.
  • Microsoft Forms is not approved for human subjects research.
  • Microsoft Forms is not approved for the collection of payment information.
  • Microsoft Forms provides opportunities to integrate the data collected into Microsoft Teams/SharePoint and Outlook.
  • Microsoft Forms provides the ability to own forms as a department/team instead of connecting ownership to individuals.

QuestionPro: Web-based survey tool for general use.

  • QuestionPro is not approved for the collection of highly confidential data.
  • QuestionPro is not approved for human subjects research.
  • QuestionPro is not approved for the collection of payment information.
  • QuestionPro should not be used for the long-term storage of data. Users should delete recorded survey response data from QuestionPro after surveys have been completed. Users who fail to download and store survey responses risk potential data loss. Downloaded survey response data should be stored in accordance with the University Data Storage Standard.

Qualtrics XM: Web-based survey tool for specialized research or the approved collection of highly confidential data.

  • All Research Data to be collected in Qualtrics XM involving human subjects (including anonymous, PII, or highly confidential research data) requires prior IRB approval.
  • Highly Confidential Non-Research Data collected in Qualtrics XM requires prior Data Manager
  • Qualtrics XM is not approved for the collection of payment information.
  • Qualtrics XM should not be used for the long-term storage of data.  Users should delete recorded survey response data from Qualtrics XM after surveys have been completed. Users who fail to download and store survey responses risk potential data loss. Downloaded survey response data should be stored in accordance with the University Data Storage Standard.

Who can use this Service?

JMU faculty, staff, students, and affiliates may use survey software.

Microsoft Forms:
Log in to https://forms.office.com using your JMU eID, password, and MFA.
QuestionPro:
Log in to https://jmu.questionpro.com using your JMU eID, password, and MFA.
Qualtrics XM:
Log in to https://jmu.qualtrics.com using your JMU eID, password, and MFA.

QuestionProUsers can access QuestionPro support 24x7. Access support using the following methods:

Qualtrics XM: Users can access Qualtrics Support 24x7. 

  • In order to contact a support representative, visit the Qualtrics Support Portal and click "Login" at the top of the page. To log in, click the "Sign in with SSO" link.
    • On the next page, enter the Organization ID "JMU" and click the "Continue" button. You may then log in using your JMU eID password, and MFA.

Are there resources for migrating my existing surveys between platforms?

How do I store and archive data?

Microsoft Forms, QuestionPro, and Qualtrics XM should not be used for long-term storage of data. Users should delete recorded survey response data from the platform after surveys have been completed. Users that fail to download and store survey responses risk potential data loss. Downloaded survey response data should be stored in accordance with the University Data Storage Standard.

Liaison librarians in JMU Libraries can help you with your data management plans, teach workshops on best practices, and help identify repositories to find and deposit data. In addition, IT consultation services are available to help guide individuals with data handling and storage to suit their needs and to meet specific regulations and compliance requirements. To request such a consultation, submit a Technology Solution Request (TSR) and you will be matched with an appropriate advisor.

Can I anonymize survey responses?

  • Microsoft Forms:
    • Yes, in the form settings there is the option for collecting anonymous responses. Navigate to Settings and the section Who can fill out this form. Select Anyone can respond: anonymous response, doesn't require sign-in.
  • QuestionPro:
  • Qualtrics XM:
    • Yes, by using Anonymous Links, Qualtrics XM will not collect respondent details.

What data export formats are available? 

  • QuestionPro: Word, Excel, PowerPoint, SPSS, CSV, SQL
  • Qualtrics XM: CSV, TSV, Excel, XML, SPSS, and more

Can I collaborate with colleagues?

  • Microsoft Forms:
    • Yes, click on the ellipses (more form settings) in the top-right corner of the form and select Collaborate or DuplicateCollaborate allows users to share the edit link to the same form and include others in the data collection. Duplicate allows users to share the form as a template for others to create their own similar form with.
  • QuestionPro:
    • Yes. To collaborate with colleagues at other universities, send a request to helpdesk@jmu.edu.
  • Qualtrics XM:

Can I use survey software tools to collect personally identifiable information (PII) or highly confidential data?

  • Microsoft Forms:
    • Microsoft Forms is not approved for collecting highly confidential data.
    • Microsoft Forms is not approved for human subjects research.
  • QuestionPro:
    • QuestionPro is not approved for collecting highly confidential data.
    • QuestionPro is not approved for human subjects research.
  • Qualtrics XM:
    • The collection of highly confidential data within Qualtrics XM requires prior Data Manager approval.
    • All research data involving human subjects collected in Qualtrics XM (including anonymous data, PII, or highly confidential data) requires prior IRB approval.
