How to Request a 1098-T Form

Current Students: Log in to Madison Money Manger (M3) and click on "Tax Forms" on the left side to access all past 1098-T forms. Authorized Users with access can log in on M3 as well to access forms. 

Former Students: If you no longer have access to your MyMadison account to access M3, you have two options to request your old 1098-T forms:

  1. If you had Authorized Users set up, they will still have access and can log in to view your forms.
  2. The former student can email ubo@jmu.edu with the following items included:
    • Student ID Number
    • Student Name
    • Last 4 digits of student's Social Security Number
    • Student's date of birth
    • What year(s) of the 1098-T form you would like sent and how you would like it sent (email, mail, etc.)
    • Signed signature on a piece of paper sent as an attachment (this is not needed if sent from your @dukes email or the email you had listed as your "home" email in MyMadison)
